Monthly Time Reports

This section includes a set of Monthly Time Reports (timesheets) for you to use in validating the necessary work hours for Cooperative Education credit. Each report also includes a brief exercise that addresses some aspect of your education or career journey. Complete the correct exercise for the month you are reporting and record the hours for each day you worked. The supervisor must sign the sheet. The completed reports will become part of your portfolio that will be submitted to your faculty coordinator at the close of the semester. If you enroll in Cooperative Education more than one semester, do not repeat any of the monthly exercises until all twelve have been done once.
